Thank you for allowing us to be part of your healthcare team! Please be advised that effective March 10, 2022, Dr. Karel Capek is no longer associated with Total Care 2 U. You may choose to remain at Total Care 2 U or, if you desire, Dr. Capek would be happy to discuss with you options to become a patient in his private practice. If you choose to remain with Total Care 2 U, no action is necessary on your part, and you will continue to receive some of the finest in-home and telemedicine physician care in the state of Texas. If you desire to transfer to Dr. Capek or to another practitioner of your choice, you may request that your medical records be released and forwarded by contacting Total Care 2 U. Depending on your needs, you may also choose to continue to be treated by both Dr. Capek and Total Care 2 U, in which case, you will still need to request that Total Care 2 U provide a copy of your medical records to Dr. Capek.
